Guardian Angels Pediatric Home Care is seeking skilled and compassionate Licensed Vocational Nurses (LVNs) to join our dedicated team. Under the supervision of a Registered Nurse (RN), the LVN will provide high-quality, individualized PDN to pediatric patients in a home setting.
Key Responsibilities
- Assist in assessing and identifying the patient’s physical, psychosocial, and environmental needs in coordination with the RN.
- Participate in the planning and delivery of care in accordance with the individualized Plan of Care (POC).
- Administer nursing care and treatments as prescribed, ensuring safety and adherence to infection control practices in compliance with OSHA guidelines.
- Ensure continuity and consistency in care delivery through accurate
- documentation and regular communication with the care team.
- Maintain open, respectful, and professional communication with patients, families, caregivers, and interdisciplinary healthcare professionals.
- Document patient care activities through clinical notes, communication logs, and collaboration with supervisory staff.
- Attend and participate in scheduled supervisory visits and training sessions to support ongoing professional growth.
- Uphold and promote Guardian Angels Pediatric Home Care’s mission, values, and policies in every interaction.
Qualifications
- Current and valid LVN license in the state of Texas (or applicable state of practice)
- Minimum of one year nursing experience preferred (pediatric or home health experience a plus)
- CPR certification (required)
- Strong interpersonal, communication, and clinical assessment skills
- Ability to work independently while collaborating with a multidisciplinary care team
- Must be able to adapt to changing patient needs and home environments
- Reliable transportation and ability to travel to patient homes as needed
